The Colorado Department of Corrections says a mistake made by their department allowed 29-year-old Christopher Moore, who had violated parole, to remain free. That oversight led to a deadly police chase Tuesday, ending in a head-on crash that killed 51-year-old Steven Ainsworth, an innocent driver. Moore also died, and a woman in his car was injured.
